Register with our practice

You can now register with our practice online, which saves you coming to the surgery and completing paper forms. Please follow the following link:

If you would prefer the paper route, please speak to the receptionist and arrange to collect the relevant forms for completion. or click here to download the registration . Although you will be registered with the practice and a specific GP you may specify a preference for a particular doctor if you wish. Return all completed registration documents to the practice reception team.  They will ensure that your documentation is processed, check your eligibility to join and add you to the practice list.

We are sorry but at present, we cannot accept temporary patients. There are exceptions for young babies or palliative patients.

Named GP

As part of the new GP Contract all patients now have a named accountable GP. That GP will have overall responsibility for the care and support that the surgery provides to you. They will also work with other relevant health and care professionals, who are involved in your care, to ensure that your care package meets your individual needs.

This does not mean that you will always have to see the same GP, you can see any GP at your request.  However, the named GP will be ultimately responsible for all your medical needs.
